Responsibilities (Key deliverables and tasks)
- To conduct dietetic assessments and develop care plans for nursing home residents who have been referred to Nualtra
- To triage all referrals in the allocated territory and ensure that residents are reviewed within a clinically appropriate timeline
- To provide nutritional support and advice on the Nualtra product range to health care professionals (HCPs) and their patients
- To support nursing homes to deliver the best nutritional care to their residents e.g., conduct online or onsite training with staff in nursing homes
- Liaising with territory partners to manage relationships with key stakeholders and HCPs in the region
- To collaborate with colleagues to develop support materials and resources for HCPs
- To deliver projects as a member of the Nualtra Team and HCP teams to drive innovative clinical solutions to partner nursing homes.
